Participating Employer Documents
Please contact the Administrative Office for a copy of Trust Agreement, Summary Plan Document and WRAP document.
These documents are available to participating employers only.
All documents (including binder check) must be received by the 20th of the month prior to the 1st to assure coverage.
1. Small Manufacturing Association of California Application – Application to join SMA. Membership with SMA is mandatory in order to join the CSMHWTF.
2. Adoption Agreement – Completed and signed by company officer.
3. Addendum to the Adoption Agreement – Completed and signed by company officer.
5. Group Application – Completed and signed by company officer.
6. Participating Employer Checklist – Binder check totaling amount for employee/plan designation must accompany this document.
7. Payroll Record Review Procedures – Quarterly Contribution Return and Report of Wages for all employees (full and part time).
